Ethics Commission

Documents necessary for the assessment of the study within the local ethics commission of the Hospital Jihlava:

• An application containing the title of the study, the study identification number, and the name of the examiner.
• Protocol and its amendments (2x).
• Curriculum vitae of the examiner (or documents confirming the examiner's qualifications).
• Data required for invoicing (address of the contracting authority, ID number, VAT number).
• Address of the multicentre ethics commission reviewing the study.

The request and materials can be addressed to the EC Secretary - see Ethics Commission members.
These materials will be discussed at the next EC meeting (if delivered at least 2 weeks in advance).
There is a fee for the client for the assessment of the study 15 000 CZK – see Price list of Hospital Jihlava.
If you are unsure, you can contact the Chairman or the Secretary of the EC.
These rules apply to the local multicentre commission.
