Information for the bereaved

Dear Madam, dear sir, let us express our deep regret for your loss.

Here is some practical information that may help you with the necessary formalities.

Practical information 

Discuss the organization and form of the funeral with your family,
if you are considering a church funeral, contact the relevant clergyman before visiting the funeral service,
call the office of the funeral service you have chosen and arrange an appointment.
you are entitled to the paid leave—two days for making arrangements and one day for the funeral,
prepare clothing for the deceased and the following documents:

  • body inspection report;
  • your ID card;
  • birth and marriage certificates of the deceased person (if you have them);
  • photograph (if you want it on the obituary card).

It is advisable to involve children sensitively, give them space to ask questions, and allow them to decide whether they want to attend the funeral.

Arranging a funeral

The bereaved are entitled to choose a funeral service provider. Hospital staff are not authorized to provide information about funeral services, and the hospital does not arrange funerals. Funeral service providers can be selected at the following website: www.pohrebnictvi.cz  or by visiting one of the funeral services of your choice. For the purposes of arranging the funeral, the department of the hospital where the patient died will issue the funeral organizer with a so-called body inspection report upon presentation of their identity card.

Where to submit the following documents, if applicable:
  • identity card – to the nearest municipal office with extended powers or registry office;
  • health insurance card – to the health insurance company;
  • driver's license – to the traffic inspectorate;
  • passport – any registry office in the Czech Republic, the Police, or a diplomatic mission;
  • military service record book – to the district military administration;
  • firearms license – Police;
  • residence permit, confirmation of temporary residence in the Czech Republic, permanent residence permit, residence card of a family member of a European Union citizen, or permanent residence permit of a European Union citizen – to the nearest Police department
  • If the deceased received or had applied for a care allowance, it is necessary to inform the relevant Work Office.

The loss of a loved one is one of the most difficult experiences in life. Everyone experiences grief differently, and it is natural that it lasts for different lengths of time.
